Correction of Class II Malocclusion with severe crowding on non-extration basis using clear aligners with mandibular advancement technique

A 15-year-old patient presented with a severe Class II malocclusion, characterized by a retrognathic mandible in relation to the maxilla, a pronounced overjet measuring 9 mm, and severe dental crowding.

Following a precise diagnosis and comprehensive treatment planning, we opted for a non-extraction approach using clear aligner therapy. Without the removal of any teeth, we successfully managed the case through strategic arch development and functional correction.

Space was created by expanding the dental arches and destabilizing the posterior maxillary teeth, which allowed for effective crowding relief. To address the excessive overjet and mandibular deficiency, a mandibular advancement technique was employed. This approach facilitated significant improvement in the sagittal discrepancy, achieving a functional and esthetic occlusal relationship.
