Treatment for Mandibular Deviation and Facial Asymmetry

This 34-year-old female patient sought orthodontic care due to noticeable facial asymmetry and functional discomfort caused by a significant mandibular deviation to the right. A comprehensive clinical assessment revealed a Class III malocclusion, crowding in both arches, and considerable occlusal imbalance.

Our treatment objectives focused on correcting the mandibular deviation, achieving proper dental alignment, and enhancing facial harmony—all without surgery or extractions. Clear aligner treatment for mandibular deviation and facial asymmetry was chosen as a minimally invasive and highly effective solution.

The orthodontic plan involved the expansion of both arches using clear aligners, along with interproximal reduction (IPR) and molar distalization to relieve crowding. Intermaxillary elastics were introduced to improve canine and molar relationships and to coordinate the arches.

Over the course of approximately 24 months, the patient achieved significant improvements in facial symmetry, dental alignment, and occlusion. The final outcome featured stable Class I molar and canine relationships and a more harmonious facial profile. The patient also reported increased confidence and improved function in daily activities.

To preserve these results, a lifelong retention protocol with both fixed and removable retainers was implemented.

This case highlights the power and precision of clear aligner treatment for mandibular deviation and facial asymmetry, demonstrating that even complex skeletal cases can be effectively managed without surgical intervention.
